Self-Pay and Out-of-Network Rates
$250 per initial assessment (60-80 minutes) $200 per individual session (45 minutes) *Payment is due at the start of every session
Insurance
I am currently in-network with Magellan Health Services of California, Inc., which often provides mental health services for individuals covered by Blue Shield of California health insurance. I will verify whether or not I am a covered provider for you prior to scheduling an appointment. As an out-of-network provider for all other insurance companies I am happy to provide a super bill which you can submit to your insurance company for possible reimbursement of services. Please be aware that your insurance company determines whether or not to reimburse and, if so, the amount that will be reimbursed. Also, please note that insurance companies require diagnoses be included in super bills.


Cancellation Policy
Since scheduling an appointment involves the reservation of time specifically for you, a minimum of 24 hours’ notice is required for cancelling or rescheduling an appointment.
If you cancel or reschedule an appointment with less than 24 hours’ notice, you will be responsible for paying the full session fee.
If your insurance provider does not cover such fees, you will be responsible for ensuring they are paid.
